I always gave my cats outside access and they would follow me on long hikes for even miles. I had to move suddenly and took refuge over the winter in a cabin; the coyotes howled outside, and yet my cats begged me at the door to let them out. It was their choice and I honored it. Neither one came back. They would have been very unhappy as I had to move more before finding a new home.
Now my new cats are my best pals, and i even believe they are little kitten angels came from the same Mother Cat Star Ship, a group soul as it were, which has repeatedly sent me reincarnations old all my old pals.
Our current cat lives for the outside. He has injured himself many times over the years and had to wear the cone as a consequence. When confined indoors he is miserable and does his best to pass on his misery to us. We have friends in Barcelona whose cat is a rooftop cat. Or he was, until he spent a week trapped in a garage. He is now confined and is truly miserable. I am of the opinion that all cats need the outdoors. There are risks associated with letting them out, just as there are risks to the cat's mental health keeping them in. It's a fact of life with an outdoor cat that one day you may never see them again, and that would break my heart. However, keeping an outdoor cat indoors would break it more.
All cats need out, but more safely. For example: cabin with coyotes - coyote proof fencing attached to the cabin window for a catio (it does exist, limited to day time outside. Mine have coyotes here, and yet many cats are roaming. Mine in the yard, but at night we close the pet door, open up the screen in porch and let them get their nature there. Some cats will walk on leashes. Diablo my heart cat, walked just like a dog on a leash all around the urban wilds of Crystal City - he caught a bird and several mice while in the city 'jungle'. The concierge love to watch as we'd stroll out the entrance for our nightly poop walk (he waited to poop for that walkies, ah the stories). We moved away from the urban jungle to sub urban, where I had a town home located beside a 1 mile round lake - with land buffers. Cat was gloriously happy rabbits were his game of choice and dogs learned quickly to respect his bit of the sidewalk. Once day he roamed at age 10 was likely hit by a car crossing the road or parking lot. Disappeared for 24 hours. Came back nearly dead, but survived and lived to 18. Shrugs. He was smart and until his brain went gaga (they get dementia like humans) and arthritis he was allowed to roam. But then I confined him to the back yard and he was fine, he couldn't climb out anymore. For many years I had a cat enclosure for Diablo and for my other street cat feral, so th ey could be outdoors in all weather (heated cat shelters) and only come in if they wanted ... my cats were very happy and all lived long lives despite the dangers.
Fortunately, in the UK, the most dangerous animal my cat is likely to encounter is a badger. Standard European, not honey. Foxes are common, but the non-urban ones keep themselves to themselves. I haven't seen one for a while. Unofficially, there are big cats roaming in the South West, but they are rarely seen.
Sounds like his brain is inflamed from a fall. Maybe a homeopathic remedy from the Vet? Your loving attention will be most appreciated by Alex. Hope he's better soon. Sending prayers for Alex and you!
Yes. After moving my dog to another country I learned the hard way that there are things they might be exposed to that they are not in their original place. Animals seem much more susceptible than humans.

Alex may have a concussion, or may have been terrorized by an anti-cat neighbour. Cats have great resilience & "nine lives". Alex is loved and in the best of hands whatever the outcome.
Sending healing thoughts and prayers but I wouldn't let them roam up there anymore especially if no one is around. My cat ran out of the house undetected and was hit and killed by a car several years back.

Make sure the vet checks his ears for mites - if that’s a thing there, and they’re bad enough it could cause balance issues.
